Preparing for your first appointment
For your convenience, all of the paperwork you will need for the new patient registration process is available on our website or we can mail you a packet. Please complete, print, sign the required forms, and return them to us prior to your first appointment. We will request your digital signature on acknowledgements of our privacy practices, financial policy, and Conditions of Admission at check in.

Arriving at your first appointment
If you have not completed the forms ahead of time, we ask that you arrive at least 30 minutes early to allow time to fill out the forms. When you come to your appointment, you will need to bring your insurance card(s) and a photo ID such as a driver's license. If you are not the insured person, you will also need to bring their information. We require payment for any co-pays by insurance at check in.

What is the Patient Portal?
The Patient Portal is your online gateway to accessing medical information and communicating electronically with CVH Clinic. You can view summaries of your visits, lab and imaging results, medications and more. The Portal offers a way to securely send and receive messages with CVH Clinic. You can also request and view upcoming appointments, all while keeping your information safe.

CVH Clinic Services:

Family Health Care - newborn to end of life: Well check ups or family care. We want our patients to know that we offer care for every member of the family from newborn, adolescents to adult and the elderly. We offer sport physicals, Well Child checks, and Adolescent and Adult physicals along with CDC age appropriate vaccinations. CVH Clinic provides the mandated schedule of well child checkups where the baby is seen every few months up until 2 years of age and then yearly after age 2.

After 2 years of age, we encourage and recommend an annual check up to update medical history, check height and weight and vaccinate if needed. Into adolescence, it is still recommended to do a checkup every 1 - 2 years. We can take care of sport physicals at this appointment as well as address other issues that impact our adolescent patients.

For adults, depending on insurance, a checkup every 1- 2 years may be recommended. For the elderly population we do recommend an annual visit to update medications, history and perform laboratory studies if needed.

Women's Health: The doctors at CVH Clinic and specifically Paavani Atluri, MD and Khriti Choudhary, MD are available for the multitude of women's care that may be requested. We perform well women's exams including pap smears and pelvic exams and breast exams. We are available to help with family planning and contraception and the procedures associated including IUD insertion/removal, Nexplanon insertion/removal and oral contraceptive pills.

All of our CVH Clinic physicians provide a full spectrum physical exam including a head to toe evaluation along with the well women checkup.

Medical & Surgical Procedures: Skin lesions, laceration repair, Colonoscopy & Upper Endoscopy, Laparoscopic & Traditional Inguinal, Umbilical & Abdominal Wall Hernia repair, Laparoscopic Cholecystectomy, Laparoscopic & Open Surgery of the Alimentary Canal, Breast Surgery, Liver Biopsy, Acute care of the injured patient, Care of Benign Ano-Rectal Disease, Biopsy & Excision of Skin Lesions. Hip & Leg fractures evaluation & surgery.

Preventive Medicine: The CVH Clinic Physicians and staff feel very strongly about our patient's wellbeing. We believe in preventive medicine and staying healthy. We offer vaccinations and well-baby checkups from newborns to adolescents and beyond. Our physicians provide a service that no other practice in town does: at CVH Clinic, we manage our patients from birth on. As one ages, we encourage regular complete physicals to help identify issues at an earlier, treatable stage.

Orthopedic: From an acute injury with sprains and strains to an acute fracture we are able to evaluate, and start treatment immediately after injury if needed. If casting or splinting is required, we have the capability to manage that as well.
